Individual Services

Individual sessions occur weekly (to begin) then can move to bi-weekly, in person or video call, depending on the needs of the client.

Clients will eventually transition to monthly or as-needed visits once you feel ready.

Short term (less than 6 months) and long-term support is available.

These sessions will be scheduled as @ $125 for 45 minute sessions.

Extended sessions are available as needed / requested.

Fee for Service

Couples or Family

As a systems focused therapist, I offer clients the option of combining individual therapy with family therapy to bring in key attachment and support folks in their life to sessions from time to time. Family systems help support the client in their work between sessions. @ $150 for 60 minute sessions.

Clients define 'family’ on their terms and family sessions may include a partner, close friend, chosen family member, or biological family member.

Couples may choose to get support to keep things flowing smoothly or when dynamics have reached an impasse. Communication techniques tend to be extremely beneficial for both parties.

$150 for 60 minute session $175 for 75 minute session (ideal to begin)

Groups

Diagnostic Evaluations

Autism Assessments are offered over a series of 3-5 sessions to explore a full picture of client’s history, current presentation of behaviors, and conduct a clinical interview to assess if a client meets the DSM-5 TR Criteria for an Autism diagnosis.

Autism Assessments can be paired with ongoing therapy services or accessed as short-term assessment-focused services if you already have an established therapist-client relationship with another professional.

Average COST Breakdown $2,500

  • Appropriate for: Children ages 2+ adolescents, adults, and elders

  • Includes: Up to 6 hours of direct testing time and up to 4 hours for records review, collateral interviews, interpretation, and report writing and up to two hours for post-evaluation report review and feedback session.

  • Payment schedule:

    • $625 (25%) down payment to secure your spot in the waiting list

    • $1250 (50%) due at the initial testing appointment

    • $625 (25%) due at the beginning of the report writing process

*Some cases may require more additional time to complete the assessment will be billed at $200 per hour. Example: attending IEP meetings with school teams to support and advocate for the student / family.

Need-based fee reduction may be available upon request and verification of financial need.

Sliding Scale

I am committed to making my services as accessible as possible.

I provide a sliding scale range for clients paying out of pocket, as I believe that therapy should be accessible to all.

Before the first session, we can discuss what price point you are able to reasonably afford based on the scale. I have a limited amount of reserved spots for sliding scale.

Payment

Payment for Diagnostic Evaluations is made on a payment schedule which is outlined on the fees page.

Payment for Therapy and Group sessions is due in full at the time services are rendered.

Payment can be made with cash, check, or a valid credit card.

Office Policies

Booking

For all services, a valid credit credit card or debit card will be required to book and hold your spot.

Some services also require a deposit at the time of booking; that information will be listed in the description if it applies.

Insurance

Please note that we do not work directly with insurance.

If you request it, we will provide a superbill (a type of health insurance-specific invoice), which you may submit to your insurance to receive whatever out of network reimbursement you are eligible for.

Need-Based Fee Reduction

Fee reductions may be available based on financial need.

If you believe you may qualify for a need-based fee reduction, please include that information in your email and Amy will discuss your situation with you to determine if you do meet criteria for a fee reduction.

Cancellation

For Diagnostic Evaluations, our cancellation policy is seven (7) days. This allows enough time to schedule another client.

For therapy and consultation sessions, our cancellation policy is one (1) business day (minimum 24 hours; business days are Monday through Friday).

If you cancel with less than the required amount of notice (“late cancellation”), you will be charged the full fee for the session you missed.

Please communicate your needs.

Reschedules

Reschedules for sessions cancelled before the minimum required time will be offered based on Amy’s availability.

No-charge reschedules for late cancellations/no-shows may be offered at Amy’s discretion, based on her availability and the client’s individual circumstances.

Payment Information

Payment is due at the time of services.

  • Clients are billed within 24 hours of services being received - unless other arrangements or payment agreements have been made during the Intake Appointment.

  • I accept debit or credit via my online secure payment system.

  • Clients may choose to have services paid for by a third-party such as a family member and this can be established during the Intake Session. Third parties are NOT granted access to clinical information, only information regarding service date, charge, and payment policies.

  • I do not accept insurance at this time. SELF-PAY only

  • Clients utilizing Out of Network Insurance Benefits via SuperBill are responsible for submitting claims on their own - unless other arrangements have been made.

Good Faith Estimate / No Surprises Act of 2022

Effective January 1, 2022, the No Surprises Act (NSA) protects you from surprise billing if you have a group health plan or group or individual health insurance coverage. It also bans: Surprise bills for emergency services from an out-of-network provider or facility and without prior authorization.

Every client will receive a Good Faith Estimate. The Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service. The estimate is based on information known at the time the estimate was created. Most estimates include the cost of bi-weekly Therapy Sessions at the standard or negotiated rate.

The Good Faith Estimate is not a contract and therefore does not require you to obtain all of the items or services outlined as available to be provided by Amy Hackmeier, LCSW. The foundation of a good therapeutic relationship between a client and therapist is the client’s right to autonomy and self-determination. Therefore, you (as the client) have the right to terminate services at any time.

The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur. If this happens, federal law allows you to dispute (appeal) the bill.

If you are billed for more than the Good Faith Estimate, you have the right to dispute the bill. You may contact the health care provider or facility listed to let them know the billed charges are higher than the Good Faith Estimate. You can ask them to update the bill to match the Good Faith Estimate, ask to negotiate the bill, or ask if there is financial assistance available.

You may also start a dispute resolution process with the U.S. Department of Health and Human Services (HHS). If you choose to use the dispute resolution process, you must start the dispute process within 120 calendar days (about 4 months) of the date on the original bill.

There is a $25 fee to use the dispute process. If the agency reviewing your dispute agrees with you, you will have to pay the price on the Good Faith Estimate. If the agency disagrees with you and agrees with the health care provider or facility, you will have to pay the higher amount.
